Billing Information

Billing Policies

  1. Self-pay patients are expected to pay before seeing the Doctor.
  2. Co-payments are due before seeing the Doctor.
  3. We will submit to most insurance companies directly. Any remaining balance must be paid by the patient.
  4. We accept cash, checks, MasterCard,  American Express, Visa and Discover.
  5. There will be a $30.00 fee incurred for returned checks.

The practice of Dr. Rick Lockhart, Middle Georgia Ear, Nose & Throat, is announcing its closure as of August 31, 2023.

Patients who need assistance in locating another ENT specialist or receiving copies of their medical records should contact Dr. Lockhart’s office by phone at (478)452-6610 or in person at 111 Fieldstone Drive, Milledgeville, GA 31061, prior to August 31, 2023.
