Middle Georgia Ear, Nose, and Throat provides all patient forms necessary to assist you with the check-in process. We ask that you print and complete the appropriate forms from our site here. You can fax these back to us or bring them with you to your appointment. Having these forms filled out ahead of time will speed up your check-in process on the day of your appointment. If you have questions about which forms to use, please call our office or you can locate these forms under the “new patient” tab.

The practice of Dr. Rick Lockhart, Middle Georgia Ear, Nose & Throat, is announcing its closure as of August 31, 2023.
